Class I-Preventive Services - Paid at $100 of the MAP (maximum allowable payable) and include:
- Initial and periodic oral examinations
- X-rays
- Test and laboratory examinations
- Prophylaxis (cleaning)
- Flouride application, emergency care
Class II-Basic Dental Services - Paid at 80% of the MAP and include:
- Space maintainers
- Restorations (fillings)
- Extractions
- Endodontics, oral surgery
Class III-Major Dental Services - Paid at 50% of the MAP after a $50 per person calendar year deductible. Major dental services include:
- Inlays
- Onlays
- Gold Restorations
- Crowns
- Prosthetic services
- Dentures and bridges
Class IV-Orthodontics Services - Paid at 50% of the MAP
- $1000 lifetime maximum
**Calendar year maximum of $1000 for Class I, II and III combined services
